The PL documents are up to date, including the changes in force from 5 January 2022, arising from the introduced Regulation REACH (EU 2020/2081). The documents are in edytowalne (Photoshop) and have space to insert your own logo and available in the . versionpdf. We are currently preparing an editable version in Word. The documents will be aktualizowane in line with the changes appearing on the PMU market.

Please note. The English version will not be fully applicable in European Union countries. It does not include the update relating to the REACH Regulation.

WYWIAD

pre-treatment consultation – thoroughly checks every contraindication to the treatment, establishes the likelihood and degree of complications and side effects, and identifies the factors that affect the result.

Agreement

client consent – carefully analysed and containing every element required in order to qualify the client and run the procedure correctly.

INFORMATOR

treatment guide – a document informing the client about how the treatment proceeds, the possible complications and the recommendations to follow before pigmentation. Extensive knowledge put down on paper, so that the client feels professionally informed

RODO

GDPR notice – the required documentation containing additional separate consents for the use of the client's image on social media and in marketing materials, as well as for training or teaching purposes.

TREATMENT RECORD

treatment record – excellent, refined so that it lays out in a practical way all the details of the technique, the pigment used, the price of the treatment, the skin type, and additional information about the main treatment and the pigmentation.

The record is divided into the following areas: brows, lips, eyes and face.

COVID-19 QUESTIONNAIRE

questionnaire – a document collecting information about current health and wellbeing, and about any illness undergone or date of vaccination.

ZALECENIA POZABIEGOWE

aftercare instructions – carefully described aftercare instructions, taking into account the client's skin type and the course of healing. The general recommendations for after the treatment and after healing are described broadly and in detail. They can be edited to match your own recommendations.
